The trigger keys off a zone, not a cost or a resolution: it fires the moment you cast a spell from somewhere other than your hand. That single constraint is the whole personality of the card. A 1/1 for two does nothing in the ordinary draw-play-repeat game, because that game never casts from anywhere but the hand. It wakes up only in a shell that launches spells out of exile, out of the graveyard, off the top of the library, wherever the hand isn't. Each such cast then does three things at once: two damage to a player, a discard-then-draw imposed on them, and a counter on the worm. The rummaging clause is the sneaky part. It reads as a hit on the targeted player, but you can point the ability at yourself, turning what looks like a penalty into a way to loot toward the next off-hand cast, so the engine can partly feed itself. The damage-and-counter half rewards raw volume, converting a stream of flashback, foretell, cast-from-exile, and impulse-draw casts into a slow drip of reach that also grows a real threat. It is a narrow payoff that reduces the whole exercise to one question (how am I casting things without paying from hand?) and rewards you only when the answer is "over and over."
