A punisher card aimed at the one decision combat is supposed to force on your opponent. Most aggressive enchantments tax attacking or punish the controller for fielding creatures at all; this one inverts the angle, charging the defender a point of life every time they commit a blocker. The design intent is friction on the defensive choice itself: it does not stop the block, it does not buff your attacker, it simply makes the safe play cost something. The trouble is that the safe play, taking damage instead of blocking, is exactly the out it leaves open. An opponent under no pressure can decline to block and pay nothing, so the enchantment only bites when the board state already favors the aggressor and the defender has no choice but to trade. That makes it a deal-closer rather than an engine: pressure built elsewhere, with the chip damage tightening the screws once an opponent is forced to interact. Red has dabbled repeatedly in this corner, taxing the opponent's combat math without touching the cards in play, but the corner has largely stayed dabbling: the payoff is contingent on a game state you have to manufacture entirely on your own, which is a tall order for a two-mana enchantment that contributes nothing toward building it.
